Dune Medicine requires completed forms returned to the office at least 2 weeks prior to your appointment . This allows Dr. O'Dell to review your medical concerns to make sure she can meet your healthcare needs. Dune Medical is unable to hold and confirm your appointment without all forms received. Forms can be printed, filled out, and scanned; or filled out online. They should be emailed to Dr. O'Dell once completed.
